Reassessment tracking
Automatically computing and monitoring when each patient is due for a scheduled clinical reassessment, and who is responsible.
Many care programs require patients to be reassessed at defined intervals — a cognitive screen every six months, a functional assessment annually, a medication review after any change. Reassessment tracking is the practice of deriving those due dates from program rules and the patient's history, surfacing what is due or overdue, and assigning ownership so nothing waits on someone remembering.
Beyond compliance, consistent reassessment is what makes a patient's record useful over time: each on-schedule assessment is another point on the trend line that progression analytics depends on.
Why it matters to our work
Automated reassessment tracking is one of the first things we deliver, because it gives staff time back immediately and builds the longitudinal data that later analytics require.
